The debate over health care reform has focused on what kind of reform we should have. In this article, the author argues that we must move toward the creation of a new health care safety net rather than a new entitlement. The existing entitlement, Medicare, is an efficient insurance plan. It has done well in providing for the health of most of the elderly population, and its innovations in the way it pays for care have provided a model for future programs. The Medicaid program is more commonly associated with the care of low-income children and the poor, and it works. It, too, has unique features that have informed subsequent programs and that make it less a program for the poor than it is a program of last resort. To become a program for the poor, Medicaid must be expanded to include coverage of all individuals with incomes below 200 percent of the federal poverty level and to provide health care services for children and for adults who have incomes above 200 percent of poverty. In this way, Medicaid can become a model program for the new health care safety net.Hypercalcemia in patients with ovarian cancer. Hypercalcemia is a common complication of advanced ovarian cancer. However, there is conflicting evidence in the literature regarding the magnitude of the problem, the patient characteristics, and treatment options available. This article reviews the incidence, risk factors, and treatment options for hypercalcemia in ovarian cancer. At the same time, it reviews evidence from randomized clinical trials of the effect of calcium and vitamin D supplements on overall survival. Data from randomized trials indicate that calcitriol-like compounds (e.g., paricalcitol) are superior to placebo and do not prolong overall survival in advanced ovarian cancer. In addition, there is little to no effect of calcium and vitamin D supplementation on overall survival. Treatment with calcitriol and paricalcitol is associated with rapid improvements in performance status, relief of bone pain, and modest improvements in quality of life.
